Remote Patient Monitoring Program for Islanders with COPD or Heart Failure

 

Getting better control over your COPD can be challenging. A new program is available for Islanders.

This new, free program is available for people on PEI who have COPD or heart failure. This program if for you if you want to:

  • better manage your COPD to improve your quality of life,
  • avoid unnecessary trips to hospital, and
  • catch problems before they become an emergency

You do not need a doctor's referral to become involved, you can self-refer by calling 902-620-3260 or emailing remotepatientmonitoring@ihis.org.

You can also have your doctor or nurse practitioner refer you to the program.
